Next-Gen Obesity & Incretin Therapies: Beyond Basic GLP-1s

Medically reviewed by Irina C. Delgado Varela, MD — Board-Certified Endocrinologist

GLP-1 medicines and tirzepatide are part of an individualized approach to obesity care. Retatrutide is investigational and is not FDA-approved as of this editorial update; access is through clinical trials. The research discussed here does not establish that an investigational treatment is available at our clinic.

Research on Brown Fat and Energy Use

Researchers are studying how incretin medicines affect appetite, fat use, and energy expenditure. Animal findings about brown adipose tissue cannot be assumed to explain weight loss in humans. A human tirzepatide study found increased fat oxidation but did not find reduced metabolic adaptation; these mechanisms remain an area of research.

Sarcopenic Obesity & Protecting Lean Mass

Weight loss can include lean tissue as well as fat; lean mass is not the same as skeletal muscle. Nutrition assessment and appropriate resistance activity can support function during treatment. Protein targets, supplements, and exercise should be individualized with your clinician or dietitian, particularly when kidney disease or other medical conditions are present.

MASH / MASLD — Liver Health and Fibrosis Risk

MASLD and MASH require assessment of liver health and fibrosis risk. A phase 2 tirzepatide trial in selected patients with MASH and moderate or severe fibrosis reported improvements in liver-biopsy outcomes. This is not a guarantee of fibrosis reversal for an individual patient. Your clinician determines which tests and treatment options are appropriate.

Rebound Weight Gain — What Prevents It

Weight regain can occur after treatment ends. In the STEP 1 extension, participants regained about two-thirds of the weight they had lost in the year after semaglutide 2.4 mg and lifestyle intervention were stopped. This finding is specific to that study, not a prediction for every medicine or patient. Discuss long-term follow-up with your clinician; do not stop or change medication doses on your own.

GLP-1 Daily Protein Estimator

Illustration = reference weight in kg × 1.2–1.6 g/kg/day; pounds × 0.453592 = kg. This is not a prescription. Reference weight and protein needs must be individualized, especially with kidney disease; ask your clinician or dietitian.

Frequently Asked Questions

Is Retatrutide FDA-approved?
As of this editorial update, retatrutide is investigational and not FDA-approved. Clinical trial findings do not establish that it is appropriate or available for routine care, or superior to another medicine in a direct comparison.
How much protein should I eat on a GLP-1?
Your clinician or dietitian should set a target using an appropriate reference weight, nutritional assessment, medical conditions, and treatment goals. The calculator illustrates arithmetic; it does not prescribe a personal intake.
